    By accessing or using the PeakHealth MT website and telemedicine services, you agree to be bound by these Terms of Use. If you do not agree to these terms, please do not use our services.

    Eligibility

    To use our services, you must:

    • Be at least 18 years of age
    • Be a resident of Montana (or other states where we are licensed)
    • Provide accurate and complete information
    • Be capable of forming a binding contract

    Medical Services

    PeakHealth MT provides telemedicine medical services for weight management. By using our services, you understand and agree that:

    • Our services do not replace your primary care provider
    • Treatment recommendations are based on the information you provide
    • Not all patients will be candidates for medication therapy
    • Results vary and are not guaranteed
    • You should seek emergency care for urgent medical conditions

    User Responsibilities

    You agree to:

    • Provide accurate and complete health information
    • Follow your treatment plan as directed
    • Report any adverse effects or concerns promptly
    • Keep your account information secure
    • Attend scheduled appointments or cancel with reasonable notice
    • Pay for services as agreed

    Payment and Billing

    You agree to pay all fees associated with your treatment plan. Fees are due at the time of service unless otherwise arranged. We reserve the right to change our fees with reasonable notice. Refund policies will be communicated at the time of enrollment.

    Limitation of Liability

    To the fullest extent permitted by law, PeakHealth MT shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of your use of our services. Our total liability shall not exceed the amount you have paid us in the twelve months preceding the claim.

    Termination

    Either party may terminate the provider-patient relationship with reasonable notice. We reserve the right to refuse or discontinue services at our discretion, with appropriate notice and transition of care when medically necessary.

    Governing Law

    These Terms shall be governed by the laws of the State of Montana, without regard to conflict of law principles.

    Changes to Terms

    We may update these Terms from time to time. We will notify you of any material changes by posting the new Terms on our website. Your continued use of our services after such changes constitutes acceptance of the new Terms.
